Service Restored on 12/21/2023 at 4:55 PM: OnBase Forms are now available. UIS has identified and addressed the root cause and service has been restored.
Service Issue Reported on 12/21/2023 at 4:07 PM: OnBase forms are experiencing a service interruption and not functioning as expected. This is affecting all users on all campuses and there are no known workarounds at this time. UIS is working to restore service and will provide more information as it becomes available.
