ICT Accessibility Review (Form C)

The procurement of Information and Communications Technology (ICT) products and services must comply with the University of Colorado Boulder's digital accessibility policy and standards. As a provider or supplier of ICT products and services to the University of Colorado, you are required to provide information and assurance that your products and services allow for accessibility and usability as established in this policy and related standards.

In order to assess and document the level of compliance, completion of this information by an authorized representative of the supplier organization will provide the University of Colorado Procurement Service Center and the campus affiliates it serves with knowledge regarding the level of compliance and satisfaction of this policy and related standards with respect to the offered products and services.

After completion and delivery of this form, a member of the Accessibility Review team may contact you to obtain additional information or clarification regarding the nature of your services and products and any perceived or material accessibility gaps.
